* Please note - I have stopped accepting Medicare in my practice. If you choose to utilize your Medicare benefits, you can self-submit the claim and Medicare will reimburse you at the out-of-network rate.
What to Bring
Prior to your first session (intake), I require documentation of insurance (if applicable), as well as the insured's date of birth. I also use a structured clinical interview for the diagnostic session.
